Patient-Centered Medical Home: What the Blues are Doing

On behalf of the 39 Blue Cross and Blue Shield companies, in October 2007, the Blue Cross and Blue Shield Association joined the Patient-Centered Primary Care Collaborative (PCPCC), a coalition of major employers, non-profit quality improvement organizations, governmental entities, consumer advocate groups, physician professional societies, and other health benefits companies whose mission is to translate the principles of the PCMH into results.  In 2008, many Blue Cross and Blue Shield companies across the country will be developing pilot demonstrations with participating physician practices to determine the best ways to implement the PCMH model at the practice level.
