How to Find Your Member ID

• Your member identification number is located on the front of your card, below your name.
• Depending on your local company, it may have a different label, such as a subscriber id or identification number.
• If you need immediate attention, call the customer-service number located on the back of your card.

Search By Location

Don't have your card handy? That's OK. We can use your zip code to look up your local Blue Cross Blue Shield provider.
